FOIA Request Form
must be filled out when requesting a copy of public documentation. All
requests will be answered within the time frame alloted by the Sate of
Illinois (varies by amount/type of documentation).
Medical Record Request:
A Release of Medical Records form must be filled out by the Patient listed on
the Report (If under 18 years old, by a parent or guardian) before records can
be copied. A photo id is required upon pick-up of any records.
Ambulance Billing:
RealEstate Taxes simply do not cover all of the expenses related to providing
ambulance service and thus we invoice Residents and Non-Residents for the
remainder of our cost.
If you have recently been transported and have not provided your insurance,
Medicare or Medicaid information, please fill out the following form so that we
may collect from them.
